Fellow Logs

All fellows must use the online surgical logs available within the ASTS Academic Universe to record their surgical experience during fellowship.


Upon completion of your fellowship, download and complete the Fellowship Completion Form and return it to the ASTS National Office along with your final surgical log.  This form must be completed by all fellows and signed by the Program Director in order to receive a Certificate of Completion from an ASTS accredited fellowship training program. Fax materials to: 703.414.7874 or email it to asts@asts.org. Please allow 6-8 weeks for certificates of completion to be reviewed by the ASTS Fellowship Training Committee and mailed.

In order to receive a certificate of completion of an ASTS accredited Abdominal Transplant Fellowship Training Program a fellow must perform a minimum number of transplants in the Principal Role over the course of the two-year fellowship for each particular organ the program is accredited to train. 

"Principal Role" does not suggest that an Attending Surgeon was not present during the procedure.  The Fellow can act in the Principal Role when working together with a Staff Surgeon.  "Principal Role" does require that the Fellow was present for the vast majority of the procedure. 

Only one Fellow can be considered to have played the principal role for any one procedure.

NOTE:  The Surgical Log and Fellowship Completion Form are all required in order to receive a certificate of completion.  Requests for a certificate should be submitted within 90 days of completing the fellowship.
