Dear Applicant,

As you complete the online membership application, please note that every field in step one except Middle Name, Assistant's Name and Assistant's Email must be completed in order to proceed to step two. 

At the end of each step, you have the option to either SAVE the application and return at a later point to complete it or continue to the next step.  If you choose to SAVE the application, you must record the number issued to you in order to complete the application.  If you lose this number, you will have to start the online application process from the beginning.  If you would like to print a copy of your application, you must print each step separately before proceeding to the next step.  

In order for your candidacy to be considered, you must complete this online application, obtain one letter of sponsorship and two letters of endorsement, and provide an updated curriculum vitae.
