By accepting this invitation, I hereby:

1. Agree to participate in the session(s) as listed in my invitation letter.
2. Agree to notify ASTS immediately if an emergency prevents me from fulfilling my role.
3. Warrant that, to the best of my knowledge, my content does not violate any proprietary or personal rights of others (including copyright or privacy rights), is factually accurate, and contains nothing libelous or unlawful.
4. Authorize ASTS to use my name, likeness, photograph, and/or biographical information in connection with the use and promotion of the program and my content.
5. Authorize ASTS to capture my content (video and/or PowerPoint presentation) for use after the meeting as a webcast or other enduring material.
6. Understand that, in accordance with the Physician Payment Sunshine Act, manufacturers of pharmaceutical drugs and devices, as well as group purchasing organizations, must report payments or transfers of value (such as meals, honoraria, or travel reimbursements) made to U.S. physicians. Some manufacturers who provided support may require ASTS to provide information about which physicians participated as faculty for this activity. I understand that if a manufacturer requests information from ASTS for reporting purposes, ASTS will provide my name, business address, specialty, NPI, and license number, as well as the amount of the transfer of value provided to me (if applicable).
7. Agree to inform ASTS if I am contacted by a third-party company or industry agent requesting my participation in the development of an educational enduring product related to this activity. ASTS will create any enduring content based on this activity and will not license any other party to do so.

By signing the ASTS Faculty Letter of Agreement and based on mutual consideration, I hereby understand and agree to the above conditions as listed.
